If you are enrolled in a non-CHED Priority Program, you have to write CHED Caraga :
1) if you will not accept the offer (waive)
2) accept the offer but will shift to a CHED Priority Program in the second semester.

1. Loan Agreement Form, attach photocopy of GSIS/SSS IDs of two guarantors/co-makers
2. Profile Form, with a 1x1 ID picture
